A senior care concierge is a professional who provides personalized guidance, coordination, and advocacy for older adults and their families. Unlike traditional home care agencies, we don't provide hands-on personal care — instead, we serve as your expert navigator through the complex healthcare and aging services landscape, ensuring your loved one receives the right care at the right time.
We do not provide hands-on personal care services such as bathing, dressing, or medication administration. Our role is as a care coordinator, advocate, and concierge — we assess needs, create care plans, connect families with vetted home care providers, and oversee the quality of care being delivered.
We primarily serve families throughout North Carolina. For families with loved ones in other states, we offer remote care management, virtual consultations, and coordination services to help you navigate care from a distance.
No. Extended Living Solutions is a senior care concierge and care management firm, not a home care agency. We do not employ caregivers or provide direct personal care. Instead, we assess your needs, recommend and vet appropriate providers, and oversee the care being delivered to ensure quality and consistency.
We begin with a complimentary 30-minute consultation — by phone or video — to learn about your situation, answer your questions, and determine how we can best help. There's no obligation. If we're a good fit, we'll outline a personalized service plan and next steps. Simply call 833-235-7552 or use our online scheduling form to get started.
Yes — medical appointment accompaniment is one of our most valued services. Your personal Registered Nurse would attend appointments with your loved one, help communicate with physicians, take detailed notes, ask informed clinical questions, and ensure you fully understand diagnoses, treatment plans, and next steps.
Absolutely. Many of our clients are adult children who live outside North Carolina and need a trusted local advocate for their aging parent. We serve as your on-the-ground partner — attending appointments, monitoring care, communicating with providers, and keeping you informed so you can make confident decisions from anywhere.
In most cases, we can begin with an initial assessment within 48–72 hours of your first consultation. For urgent situations — such as a hospital discharge or sudden change in condition — we prioritize rapid response and can often mobilize same-day or next-day.
Senior care concierge and care management services are generally not covered by Medicare or most insurance plans. However, some long-term care insurance policies do include a care management benefit. We're happy to help you review your policy to determine if any reimbursement may be available.
Our services are priced based on the scope and complexity of your needs. We offer hourly rates for specific services as well as monthly care management packages for ongoing support. All fees are discussed transparently during your initial consultation — there are no hidden costs or surprise charges.
Yes, absolutely. Your first 30-minute consultation is completely complimentary with no obligation. We want you to feel confident and informed before making any decisions. This call is simply an opportunity to understand your situation and explore whether we're the right fit for your family.
A care assessment is a comprehensive evaluation of your loved one's physical health, cognitive status, functional abilities, living environment, social support, and personal preferences. Conducted by Cassandra Jones, RN, BSN, PCCN, the assessment forms the foundation of a personalized care plan tailored to your family's unique needs and goals.
Yes — hospital discharge planning is one of our core services. We work directly with hospital case managers, physicians, and rehabilitation teams to ensure a safe, well-coordinated transition home or to a post-acute care facility. We review discharge instructions, arrange follow-up care, and monitor your loved one closely in the days and weeks after discharge.
Absolutely. Choosing a care facility is one of the most significant decisions a family can make. We conduct thorough research, tour facilities on your behalf, evaluate staffing ratios and inspection records, and help you compare options based on your loved one's clinical needs, personal preferences, and budget — so you can make a confident, informed choice.
